Who We Are
A Path Back to
Community Living
The Nursing Home Transition and Diversion (NHTD) Waiver Program helps people with disabilities and older adults move out of institutional care, or avoid it altogether, and back into life in the community. It’s part of the broader Medicaid Home and Community-Based Services (HCBS) waiver, built around one goal: more independence, more freedom, and a better quality of life, with support delivered right at home.
Z Best Homecare also supports individuals through the Traumatic Brain Injury (TBI) Waiver Program, which offers similar community-based services for people recovering from a brain injury who don’t need institutional-level care but still need consistent, long-term support.

Both waiver programs offer a wide range of services designed to help participants live independently and stay connected to their community. Here’s what’s included.
Service Coordination
A dedicated coordinator makes sure participants receive the full range of services and support they’re approved for, across both the NHTD and TBI programs.
Community Integration & Peer Mentoring
Counseling to ease the transition into community life, plus the chance to connect with peers who’ve successfully made that same transition themselves.
Home & Environmental Modifications
Practical changes to a participant’s home, ramps, grab bars, layout adjustments, so it’s safe and accessible for their specific needs.
Assistive Technology & Independent Living Skills
Devices and equipment that support daily tasks, along with skills training (part of the TBI Waiver) to help participants live more independently over time.
Structured Day Programs & Nutritional Support
Meaningful, structured activities that promote community involvement, along with nutritional counseling to support each participant’s specific health needs.
Cognitive Rehabilitation & Respite Care
For TBI participants, cognitive rehabilitation helps rebuild skills affected by brain injury, while respite services give family caregivers a needed break.

Eligibility
Who Qualifies for These Programs
Eligibility differs slightly between the two waivers, but both are built for people who need a nursing-home level of care and want to receive it in the community instead.
NHTD Eligibility
New York State residents, Medicaid-eligible, either 65 or older or 18 to 64 with a physical disability, and able to live safely in the community with support.
TBI Eligibility
New York State residents, Medicaid-eligible, ages 18 to 64, with a traumatic brain injury sustained after age 18, and able to live safely in the community.
Medicaid Required
Both programs require Medicaid eligibility as a baseline qualification.
Nursing-Home Level of Care
Applicants must be assessed as needing that level of care to qualify for either waiver.

What's the difference between the NHTD and TBI Waiver Programs?
NHTD serves older adults and people with physical disabilities who want to transition from or avoid a nursing home. TBI serves people ages 18 to 64 who sustained a traumatic brain injury after turning 18. Both provide similar community-based support.
Do I need to already be in a nursing home to qualify?
No. Both waivers support people transitioning out of institutional care as well as those trying to avoid it in the first place, as long as they meet the nursing-home level of care requirement.
